     CLICK HERE to open an order form for a name patch.  Completed name patch order forms are submitted, with $12.00 payment, to the membership/patches officer at a meeting or on a ride.  Please note that patches orders received from you are forwarded monthly after the chapter general meeting to the manufacturer and completed patches are normally available by the next chapter general meeting.  You may also send a completed form with your $12.00 payment to:
